What information does HealthScreen collect, and why?

To deliver a safe, clinically meaningful, and personalised assessment, HealthScreen collects the following categories of information:

Identity information Your full name, date of birth, contact details, and emergency contact.

Medical history Current and past diagnoses, medications, previous surgeries, and family history. This is collected via your pre-consultation form prior to your appointment.

Lifestyle and health behaviours Information about your diet, exercise habits, sleep, and alcohol and tobacco use.

Clinical data Results from all assessment components completed on your assessment day — including MRI imaging, blood biomarkers, ECG, body composition, and microbiome testing.

Payment and billing information Processed securely and not retained beyond what is required for billing purposes.

Why do you need my medical history before my appointment?

Your pre-consultation form is one of the most important steps in your preparation. HealthScreen assessments begin with an in-depth physician consultation tailored to your individual risk profile. For your doctor to make clinically informed decisions — about which components to prioritise, how to interpret your results, and what recommendations to make — they need a clear picture of your health background before meeting you.

The more complete and accurate your form is, the more relevant and personalised your assessment will be. We recommend completing it at least 48 hours before your appointment.
