What is the pre-consultation form?

The pre-consultation form is an intake form that you will be asked to complete before your HealthScreen appointment. It is one of the most important steps in your preparation and one that directly improves the quality and relevance of your assessment.

What is its purpose? HealthScreen assessments begin with an in-depth physician consultation that is tailored to your individual risk profile. For your physician to make informed decisions about which components of your assessment to prioritise and to contextualise your results accurately, they need a clear and comprehensive picture of your health history before they meet with you. The pre-consultation form is how that picture is built.

What does the form cover? The pre-consultation form typically includes:

  • Personal and contact details
  • Medical history — current and past diagnoses, previous surgeries, and any ongoing health conditions
  • Current medications — all prescribed medications, over-the-counter drugs, and supplements
  • Family medical history — known hereditary conditions or health events in first-degree relatives, including cardiovascular disease, cancer, stroke, diabetes, and thyroid conditions
  • Lifestyle information — including diet, physical activity levels, sleep, alcohol consumption, and smoking status
  • Specific health concerns or goals — anything you would like your physician to focus on or be aware of
  • MRI safety screening — questions about metal implants, pacemakers, prior surgeries, and other factors relevant to imaging safety

Why is it important to complete it carefully? The more complete and accurate your form is, the more your assessment can be tailored to what matters most for you.

When and how do I complete it? You will receive your pre-consultation form as part of your booking confirmation communications. We recommend completing it well in advance of your appointment — ideally at least 48 hours beforehand — so that our team has time to review it before you arrive.
